Important Note: For all orders exceeding $1000.00 and paid using a credit card, we require our customers to fill out, sign, and fax a Credit Card Authorization Form (CCAF) to 1 928 203-9102. This form is required for every credit card that customer have on file with us. The Credit Card Authorization Form is conveniently available for download here.
